When Sperm Goes the Wrong Way: A Guide to Retrograde Ejaculation and Male Infertility

This can occur due to nerve damage or other medical conditions that affect the bladder sphincter. Retrograde ejaculation is not painful and does not impair sexual function. Still, it can lead to a lack of sperm in the ejaculate.

Diagnosis

When investigating the cause of infertility, the spermiogram is integral to the male initial diagnosis. According to the findings, we can make the first suspicion diagnosis. If there are few or no sperm in the spermiogram, one must think of the rare diagnosis of retrograde ejaculation. Detailed diagnostics are necessary to determine the cause of retrograde ejaculation.

Treatment

The treatment of retrograde ejaculation depends on the underlying cause and the severity of the condition. Medications can help in some cases to find sperm in the ejaculate. If this is unsuccessful, sperm can be extracted from the urine after appropriate pretreatment, for example, directly through a catheter from the bladder. If sperm is present, artificial insemination is an option. The sperm is directly injected into the egg to enable fertilization. This method is known as intracytoplasmic sperm injection (ICSI).
